Free Sports Physical Registration

Free sports physicals are offered to all current students in grades 7-11. PIAA requires a sports physicals for all student athletes participating in a PIAA sponsored sport. You must schedule an appointment to receive a free sports physical. The next Free sports physicals session will be offered on November 6, 2025 beginning at 3:00 PM in the Lebanon High School Gymnasium. Please use the quick links below to access the physical form, Physical registration sign-up form, and submission of your completed physical.

Physical Submission

Lebanon School District (LSD) will utilize Arbiter for submission of sports physical screening exams for all 7th through 12th grade students who try out for an LSD sponsored sport. Please click on the Athletics Registration Link above to submit your completed Physical.  The account MUST be created by a Parent/Guardian

PLEASE DO NOT UPLOAD YOUR PHYSICAL TO BIG TEAMS, as we are no longer using this platform.

• ·         School sponsored Pre-Participation Athletic Physicals are required for students in grades 7 - 12 ( SY 2025-26) who are trying out for an LSD school sponsored sport.

• Print the Health History and Pre-Participation Physical Exam form. Please complete Section 5 -- Section 6 will be completed at the physical.  

• Sections 5 & 6 are available as a paper copy in the Athletic Office
• Sections 5 & 6 are available as a download using the quick link above

• ·         All physicals for the 2025-26 school year MUST be dated May 1, 2025 or after.
